  • Original Certificate of Live Birth
  • Completed Player Registration Form. This form is available below for download, please print and fill out prior to registration

for Returning Players:

  • Updated Player Registration Form. If you are a returning player we have your information and will print it. We ask that you update the Contact and Uniform Size information if necessary.

If you live in Williston Park, you are eligible to play in the WPLL.
