This form is for registration for pre-t ball and t-ball for both boys and girls. This form is also for farms baseball, minors baseball, and majors baseball divisions. When registering your child, you only need to fill in the categories that are required (*). Proof of residency and school enrollment are asked but NOT required for registration.
